It's well documented that her and Biden didn't get on behind the scenes and given he was President and was doing most of the talking re. Ukraine I'm going to attribute most blame to him. She was given domestic issues like the border to deal with.

So a failure of the administration. I'm not saying it's all her fault and the russian nuclear threat might have been serious but the whole administration were never really behind Ukraine actually defeating Russia. I remember at one point them telling Ukraine to stop hitting Russian oil depots because it might raise inflation in America. Not great
