To be honest, I don't even know how to react to that shambles.
Clearly, significant amounts of incompetence and (bad) attitudes must be embedded throughout the structure here at all levels.
Where and how to begin to tackle that eludes me.
I suppose I'd tend toward the grassroots. Start by fixing the foundations. Which means clubs and schools.
One item that'd hold attraction to me as a starting point would be completely eradicating the practice of schools forbidding youngsters turning out for their clubs.
At least try and drag the level of kids not going to the "big" schools upward with an improved club game.
Also might harden up a few of the soft upper crust kids by having to fight off big angry culchies every other week in club games rather than other pampered boyos from Campbell College or RBAI!
