I can see this going wrong for all sorts of reasons.

Its long been an issue because of 14% Spanish first langauge speakers.

It speaks to those who have zero idea of actual demographics in the USA

I still think the NHS should give invoices for care, albeit with "balance paid" at the end.
We just have no idea whatsoever how much it costs. The NHS pays about £50 for that 8 minute GP consultation, about £70 for that 30 minutes with the physio - both are a little more than double than in USA. And that's for the easy, first contact stuff.

Hell, just look at what's happened to vet prices since they all sold up to large groups or outside investors!
7 years ago, my 45kg dog was spayed for a little under £200. In 2 weeks, my 27kg dog will be £500. Same time, same skills, same equipment, less anaesthetic, just an extra set of shareholders controlling a local monopoly.

GP hasn't worked that way since 2004

Since 2004 GP surgeries have been funded based on registered patients, with demographic adjustments for age, sex, chronic patients etc.

They earn extra for quality care, but all GP services come out of a surgery budget. They paid everything out of their budget and the surplus was their profit.

It was noticeable then that some GP services "declined" in the sense that there were fewer referrals and more "pill happy" prescribing because it protected some of that surplus and generated a bit more income to the NHS in prescription charges.

Back in my childhood (late 60s, early 70s) the village I lived in had a dispensing doctor who gave you you medication there and then, or with a phone call left your prescription on a shelf in the waiting room, people were a lot more honest then.

Therefore, to put a consultation cost, albeit marked paid, would be impossible.
